For New Patients

If you are a new patient, you need to check in 15-30 minutes prior to your first appointment to complete the necessary paperwork. If you would like to save time, you can download, print, and complete the New Patient Information Questionnaire (English)* or New Patient Information Questionnaire (Chinese)* beforehand and bring it with you.

If you would like to know how Yu's Acupuncture & Herb Center uses and discloses your protected health information (PHI) and your rights to access and release your PHI, you can download and read the HIPAA Notice of Privacy Policies*.

Should you ever need to access and/or release your PHI, you will need to print, complete, and submit the Protected Health Information Patient Access Form* to Yu's Acupuncture & Herb Center. You do NOT need to bring this form with you to your first appointment.
