For New Patients

If you are a new patient, you need to check in 15-30 minutes prior to your first appointment to complete the necessary paperwork. If you would like to save time, you can download, print, and complete the NEW PATIENT INFORMATION QUESTIONNAIRE (English or Chinese Simplified or Chinese Traditional)* and ACKNOWLEDGEMENT OF RECEIPT OF NOTICE OF PRIVACE PRACTICES* beforehand and bring them with you.

Most medical insurance plan have acupuncture benefits! If you have insurance, we highly recommend you to contact your insurance company to check your benefits and eligibility of acupuncture. You may use the Verification of Insurance Coverage for Acupuncture as the guideline when call the insurance company. We also will be very happy to verify acupuncture coverage for you prior you come in or when you come in for an acupuncture visit.

If you would like to know how Yu's Acupuncture & Herb Center uses and discloses your protected health information (PHI) and your rights to access and release your PHI, you can download and read the Notice of Privacy Practices*.

Should you ever need to access and/or release your PHI, you will need to print, complete, and submit the Protected Health Information Patient Access Form* to Yu's Acupuncture & Herb Center. You do NOT need to bring this form with you to your first appointment.

Please Note: Your appointment time is reserved exclusively for you. Missing appointments compromises the effectiveness of the treatment. Please give 48 hours notice for cancellation or rescheduling, otherwise you may be charged for the scheduled appointment if cancelled or rescheduled less than 48 hoursor or no-show.
