New Patients

If you are a new patient, you need to check in 15-30 minutes prior to your appointment to do paperwork. In order to save time, it is recommended that you download and print the “New Patient Information Questionnaire” file, and fill it up beforehand.

You also can download the HIPAA Notice of Privacy Policies to know how we use and disclose your protected health information (PHI), your rights to accesse and release them, and what you need to know about this process.
